The rapid advancement of artificial intelligence in healthcare presents both immense opportunities and significant challenges. While AI promises to enhance efficiency and reduce costs, its integration must not come at the expense of high-quality patient care or ethical considerations. To address this, R. Andrew Taylor, MD, MHS from UVA Health, and Arwen B.L. Declan, MD, PhD from Clemson University, have developed a comprehensive framework to guide hospitals in this complex process.

Their Total Mission Value framework is designed to help healthcare institutions navigate the rapid adoption of AI while keeping patient care at the forefront. This framework is particularly timely as hospitals face increasing pressure to implement AI solutions quickly, often with little guidance on how to evaluate their broader impact beyond cost savings.

Balancing Efficiency and Ethics in AI Integration

The framework is structured as a pyramid, with patient care and the patient experience at its peak. This apex is supported by a foundation of ethical considerations and a base of economic sustainability. The goal is to ensure that AI supports care providers rather than replacing them or adding to their workload.

“AI is being adopted in medicine at an unprecedented scope and velocity,” said Taylor, vice chair of research and innovation for the University of Virginia School of Medicine’s Department of Emergency Medicine. “However, hospitals haven’t had a good way to weigh these decisions holistically. Our framework provides a structured approach to evaluate AI tools based on their impact on patients, staff, and the quality of care.”

The Five Pillars of the Framework

The framework integrates five ethically grounded priorities: patient carestaff experiencehospital operationseconomic impact and education and research. Each pillar plays a crucial role in ensuring that AI tools are implemented in a way that benefits all stakeholders.

Patient-Centered Care

The top priority emphasizes patient-centered care highlighting values such as integrity, honesty, trust, compassion, and respect. This ensures that AI tools are used to enhance the patient experience rather than detract from it.

Staff Experience and Workforce Development

The staff-experience category calls for hospitals to use AI to drive workforce development teamwork, and collaboration across disciplines. This approach aims to create a supportive environment where AI augments the capabilities of healthcare professionals.

The Broader Implications of AI in Healthcare

Declan and Taylor acknowledge that AI’s potential is double-edged. While AI tools can enhance care speed, diagnostic accuracy, and cost efficiency, they also introduce risks such as bias, opacity, workforce displacement, and erosion of the patient-clinician relationship. These risks are often overlooked in cost-focused analyses.

“Hospitals are inundated with new AI tools marketed to improve healthcare,” said Declan, clinical assistant professor in Clemson University’s School of Health Research. “The challenge is to discern which tools will genuinely make a positive impact. This requires a comprehensive evaluation that considers clinical, operational, and financial dimensions, with patient care at the center of every decision.”

Ultimately, the framework aims to remind hospitals of their central, defining mission providing exceptional patient care. By keeping this mission at the forefront, the hope is that AI adoption will be both swift and trustworthy.

“Our goal is to ensure that technology helps us take better care of people,” Taylor said. “If we keep this as the primary objective, the efficiency and savings will naturally follow.”

The framework has been published in the scientific journal npj Digital Medicine and is available as an open-access article. Taylor has received a grant from Beckman Coulter to support the evaluation of a clinical decision-making algorithm called TriageGo and serves as an adviser for VeraHealth.
